Thank you for visiting our Facility Scorecard page. We believe in honesty, transparency, and accountability as we care for you or your loved ones.
The healthcare provider/patient relationship is a sacred partnership that requires open communication and active participation from all parties.
We would personally like to offer an explanation as to why our State Survey star rating is low and what we are doing to improve it.
Star ratings factor in surveys from the past three years, which sometimes means problems that have long since been fixed can still “count against” a facility.
There is also a lag time between when the surveys occur and when they are posted to the federal database, meaning a facilityʼs star rating might have been impacted by a recent survey but final results have not yet been published.
A comprehensive plan is in place to raise our rating. We continually review our results, make adjustments, and look forward to improving our scores as we move forward.
While we are not perfect, we are working hard to improve quality and provide the high standard of care our patients deserve.
