Eva was admitted to Clinton Care and Rehabilitation following hospitalization secondary to increased weakness and difficulty with balance while standing and during ambulation. As you can see, Eva has been an active participant with therapy, including PT, OT, and ST services. Eva progressed from min A to MI for all self care tasks, can now walk without assistance using a rolling walker, and has improved her cognition and safety awareness and has returned home to her prior living environment.
